What is Buccal Fat Removal?

Buccal fat removal, also known as a bichectomy, is a surgical procedure aimed at removing the buccal fat pads, located deep within the cheeks. These pads contribute to facial fullness, and their removal aims to slim the cheeks, define the jawline, and create a more sculpted facial profile. This procedure is often sought by individuals seeking a chiseled and contoured appearance.

It can be paired with botox in the masseter muscle (the muscle attached to the jaw) or dermal fillers around the cheekbones to give even more of a dramatic change.

Buccal Fat Pad Excision?

What to Expect

Typically performed under local anesthesia, Dr. Saxon carefully removes the buccal fat pads through small incisions inside the mouth. This minimally invasive approach minimizes scarring and promotes faster healing. The procedure usually takes 45 minutes to 1 hour, with minimal discomfort and swelling peaking at 3 days and gradually subsiding.

After the procedure, you will have a compressive wrap placed around your jawline to prevent fluid collections. Once the local anesthesia wears off, you may have some mild discomfort. Tylenol and/or ibuprofen is usually adequate for this. Antibiotics, a mouthwash, and steroids are prescribed to prevent injections and help with swelling.

Swelling usually peaks around 3 days and starts to subside after that. We usually recommend icing and lymphatic massage as soon as tolerated to help with this. A gua sha stone or jade roller can help give more uniform pressure during massage.

It is rare to have significant bruising since the buccal fat pad sits deep within the cheeks, but this can happen and usually settles along the lower jawline. Makeup can be worn to cover this up even during the first week of healing.

Results start to become apparent around 1 month after the procedure but continue to improve for about 6 months.

Can buccal fat excision be reversed if I'm not satisfied with the results?
While it is technically possible to reverse buccal fat excision by undergoing fat grafting or other augmentation procedures, it’s essential to carefully consider your goals and expectations before undergoing surgery and communicate openly with your surgeon to ensure a satisfactory outcome.
The goal of buccal fat excision is to create a more sculpted and defined facial contour, rather than a sunken or hollow appearance. By carefully removing an appropriate amount of fat and preserving the natural contours of the cheeks, Dr. Saxon can achieve balanced and aesthetically pleasing results.
While initial improvements in facial contour may be noticeable shortly after buccal fat excision, it can take several weeks to months for the final results to become apparent as swelling resolves and the cheeks settle into their new shape.
Since buccal fat excision is performed entirely inside the mouth, there are typically no visible external scars. However, it’s essential to follow post-operative care instructions provided by your surgeon to minimize the risk of complications and promote optimal healing.
