Morpheus8 Before Facelift Surgery in Austin
There’s a lot of conflicting information online about whether Morpheus8 before a facelift helps or hurts. Some surgeons warn that radiofrequency microneedling scars the tissue and makes surgery harder. I understand where that concern comes from.
What I’ve found in my Austin practice, when it’s done with the right settings, is the opposite. It actually helps me operate. One week made that especially clear to me, because I did two deep plane facelifts on two women with very similar skin, one who had done Morpheus8 with us and one who hadn’t.
Two Patients, One Week
The first patient lives locally. We’d been seeing her for a few months for skin treatments, including Morpheus8, to prepare her skin for surgery.
The second patient flew in from Portland, Oregon. Her skin was very similar: thin and delicate. I told her skin treatments could help her result, but there was nowhere near home she trusted to do them. So she hadn’t had Morpheus8. She’d had Fraxel in the past, which doesn’t go as deep and doesn’t do the same thing I see with Morpheus8.
Both women ended up with great results. But what I felt in surgery was different.
What I Noticed During Surgery
The Patient Who Had Morpheus8
When I lifted her skin, it was noticeably healthier and thicker. I could hold it with skin hooks much better, and because I could grasp it securely, lifting was easier.
The tissue planes weren’t affected at all. I didn’t see scar tissue or anything that got in my way. Her planes were undisturbed and straightforward to work in.
The Patient Who Hadn’t
Her skin was harder to grasp. We often hold tissue with tiny skin hooks, and with her thinner skin, the hooks wanted to pull through. I had to be much more delicate, and it took me a little more time.
In my experience, Morpheus8 thickens the dermis. That extra substance is what made the difference I felt.
Why Settings Matter So Much
Both patients had their treatments in different places, and that matters. When patients do Morpheus8 in our office, we control the settings and the depth.
I’ve found that one pass at 1 mm and one pass at 2 mm is the most you need. That treats the skin without going so deep that it disturbs the fat of the face. When the energy goes deeper into the fat and the SMAS, that’s when I’d expect the problems people worry about.
I wrote more about what can go wrong with aggressive settings in Morpheus8 ruined my skin: what can go wrong and how to avoid it.
What the Research Says
The concern isn’t made up, and I want to be fair to it. A 2026 scoping review in JMIR Dermatology pulled together the published research on radiofrequency microneedling and facelift surgery. The authors concluded that RF microneedling may cause dermal fibrosis, tissue adhesions, and changes in the SMAS that could interfere with a future facelift, and that patients should be told about this.
That review pulls together reports across many devices, depths, and settings, and much of the evidence is indirect. It doesn’t separate superficial treatment confined to the skin from deep treatment into fat. In my view, that distinction is the whole story.
On the benefit side, small histology studies support what I feel in surgery. A 2023 study in the Journal of Cosmetic Dermatology took skin samples before and after microneedle radiofrequency and found more collagen and elastin at four months. A 2025 study in Scientific Reports compared microneedle radiofrequency with microneedling alone in aged skin and looked at how each affects aging fibroblasts, the cells that make collagen.
Neither study looked at facelift surgery directly. We need better research that follows patients from skin treatment into the operating room. Until then, I’m going by what I see and feel when I’m holding the tissue.
Timing Before Your Facelift
You don’t have to stop Morpheus8 long before surgery out of fear. I think it helps even close to the facelift, as long as it’s done by the right person at the right settings. I usually recommend doing Morpheus8 at least 1 month before surgery.
If you’re traveling for surgery, try to find someone near home whose settings you trust, or plan a few visits to us ahead of time. You can also read about prepping the canvas in my post on what separates a good facelift from a great one.
Frequently Asked Questions
Does Morpheus8 cause scar tissue that makes a facelift harder?
It can if the treatment is aggressive and goes into the fat. With superficial settings confined to the skin, I haven’t seen scar tissue in the planes I operate in.
What Morpheus8 depth is safe before a facelift?
In my practice, I use one pass at 1 mm and one at 2 mm. I don’t think deeper treatment is needed for skin prep.
Is Fraxel the same as Morpheus8?
No. Fraxel is a fractional laser that works more superficially. Morpheus8 combines microneedling and radiofrequency energy and, in my experience, does more to thicken the dermis.
Can I get Morpheus8 after my facelift?
Yes. Many patients use it to maintain skin quality once they’ve healed. I usually have patients wait about three months after surgery before starting Morpheus8 again.
Should you get Morpheus8 before your facelift?
Don’t let fear keep you from preparing your skin. Ask who’s doing your Morpheus8 and what depth they’re using, because that’s what decides whether it helps your facelift or gets in the way.
If you’re planning a facelift and want a skin prep plan, schedule a consultation at my office in southwest Austin, toward Dripping Springs.
